Empowering Hope: Transforming Homeless Care Through Community Paramedicine

Biography:

Sherri is deeply committed to providing compassionate, appropriate care, with a focus on the “Right Care, Right Place” approach. Guided by the belief that “To the world you are one person, but to one person you may be the world,” Through her service, Sherri strives enhance innovative, patient-centered programs.

Abstract:

In our community, a lack of shelters for homeless men and women without children has led to an over-reliance on 911 services for medical emergencies, social needs, and respite care. In response to this growing demand, our Community Paramedic Team has partnered with a local Street Outreach program to address the complex needs of this vulnerable population. This collaboration has allowed us to build trust and provide comprehensive care, including medical treatments, mental health evaluations, substance use disorder (SUD) resources, and connections to primary care and social services. Our goal is to deliver high-quality, patient-centered care to every member of our community, regardless of their economic status. This presentation will explore the development of the program, the wide range of services we offer—such as wound care, chronic disease education, behavioral health support via telemedicine—and the significant impact we’ve made on both individuals and the broader community.
